Follow the sequence from a healthy nervous system to a stuck-open sensory gate. This is what your ENT is trained to look at (the ear) versus what's actually happening (upstream, in the brain).
A Healthy Sensory Gate Is Closing All The Time
Your brain constantly generates its own baseline neural chatter. Under normal conditions, a filter called the thalamic reticular nucleus keeps that internal noise out of your conscious perception, using GABA (the brain's primary inhibitory neurotransmitter) as the "shut" signal.

Chronic cortisol, ototoxic medications, aging, and repeated loud-noise exposure gradually deplete the raw materials the brain uses to make GABA (magnesium, B6 in its active form, glycine). GABA production falls below the threshold the sensory gate needs to stay closed.

An ENT's toolbox is trained on the ear canal, the eardrum, the ossicles, and the auditory nerve. In most tinnitus cases all four look fine, and that's why the hearing test and MRI come back clean. Your doctor wasn't lying when he said "there's nothing wrong."
Tinnitus is upstream from all of those structures. It's a neurochemistry problem in the sensory-gating apparatus of the brain, which is a completely different specialty from otolaryngology. Thalamic sensory gating and the role of GABAergic inhibition
Foundational reviews (Steriade, 2005; McCormick & Bal, 1997) established that the thalamic reticular nucleus uses GABA to filter which sensory signals reach cortical awareness. When GABA signaling is impaired, this filter fails.
Central-nervous-system origins of chronic tinnitus
Consensus reviews (Eggermont & Roberts, 2004; Rauschecker et al., 2010) established chronic tinnitus as a disorder of central auditory processing, not of the cochlea. This is why hearing tests and MRIs typically come back normal.
Magnesium, B6, and GABA synthesis
The enzyme GAD (glutamate decarboxylase) that produces GABA is B6-dependent, and its function is magnesium-modulated. Deficiency in either measurably lowers brain GABA (Kennedy, 2016; McCarty, 2016).
Ashwagandha and cortisol reduction
Randomized double-blind trials of KSM-66 extract at 300-600 mg (Chandrasekhar et al., 2012; Lopresti et al., 2019) showed 20-30% reductions in serum cortisol and improvements in sleep quality scales after 8 weeks.
L-theanine and alpha-wave activity
EEG studies (Nobre et al., 2008; Kim et al., 2019) showed 200 mg L-theanine reliably increases alpha-wave power within 30-45 minutes, correlating with reduced sleep-onset latency and improved subjective sleep quality.

Common Approaches That Fail
Why "Traditional" Tinnitus Fixes Don't Hold
Understanding why other approaches fail is part of understanding why the mechanism above works.
×White noise & masking
Masks the symptom but does nothing to the underlying GABA-gate mechanism. The brain adapts to any masking sound in about six weeks, at which point the tinnitus "returns" as loud as before, sometimes louder.
×Standard tinnitus supplements
Most category leaders use capsule delivery (85% of active lost to stomach acid) or alcohol tinctures (40% evaporation, sleep-disrupting alcohol) and dose at 1/10 to 1/20 of what the clinical trials actually used. The formulation misses the mechanism.
×ENT visits
An ENT's diagnostic toolbox stops at the auditory nerve. Sensory gating happens two structures upstream. There's no drug an ENT can prescribe to target it, which is why "learn to live with it" is the standard reply.
×Sleep medication
Sedatives knock out consciousness but don't restore GABA signaling in the sensory gate. Users typically report the ringing is still there in the moments before and after the drug takes effect, and morning ringing is often worse.
